List of Documents For economic contract between PLASKE JSC and the other Party

1.1 Legal Body – Ukrainian residents:
1.1.1. Legal Body State Registration Certificate, copy.
1.1.2. Unified State Register of Enterprises and Organizations of Ukraine Certificate, copy.
1.1.3. Payer of Value Added Tax Certificate, copy or Single Tax Payer’s Certificate, copy 1.1.4. Articles of Organization, copy (or any other Constituent Document) or extract containing the following information: full and abbreviated name of company, location, executive terms of reference.
1.1.5. Document proving the authorities of a party signing the contract on behalf of company, copy (e.g. power of attorney, order or protocol of assignment, etc.)
1.1.6. Copy of License (Certificate, Permit, etc.) if the company’s activities are to be licensed or certified according to the Ukrainian legislation in force.

1.2 Sole Proprietor:
1.2.1 Certificate of State Registration of Sole Proprietor, copy.
1.2.2 Payer of Value Added Tax Certificate, copy (if any).
1.2.3 Single Tax Payer’s Certificate, copy (if any).
1.2.4 Billing documents proving the payment of the single tax or advance payment of IT for the last reporting period, copies.
1.2.5 Passport, copy.
1.2.6 ID Code Certificate, copy.
1.2.7 Copy of License (Certificate, Permit, etc.) if the company’s activities are to be licensed or certified according to the Ukrainian legislation in force.

1.3 Natural Person – Ukrainian resident:
1.3.1. Passport, copy.
1.3.2. ID Code Certificate, copy.

1.4 . Legal Body – Non-residents:
1.4.1. Document proving status and registration of a legal body in the state (extract from commercial, trade or other registrar of the state concerned), copy.
1.4.2. Document proving the authorities of a party signing the contract on behalf of the nonresident company, copy (Power of Attorney, etc.).

2. The copies should be exemplified by the authorized representative of the company (signature and seal).

3. The documents should be translated into Russian or Ukrainian and formalized (apostilled) according to the legislation in force.
